Project Summary:
CHAI and Azeem Premji Foundation in collaboration have launched a new project “VASATI” for providing palliative care services in home environment in Hyderabad and Medchal Malkajgiri districts of Telangana State. The project is aimed at enhancing the quality of life and quality of death of the people living with terminal and chronic illness and their families through home visits, community awareness & education, and networking with different stakeholders.
Job Description
The incumbent will work in a team, be responsible for the daily operations, logistics, and communication for the Home-Based Palliative Care Program. Ensure that patients with serious illnesses receive comprehensive, high-quality, and compassionate care in their homes. Serve as the central point of contact for the interdisciplinary team (physician, nurses, nurse assistant, physiotherapist, social workers), patients, families, and community representatives. Works with local community members, institutions and organisation to bring awareness. Engages and works with local community leaders and other stakeholders in respective project sites, coordinates with various organisations to extend social, psychological and economic support to the family and the patient.
The incumbent is expected to deliver specific task or assignments to ensure the projects runs smoothly and achieve all the deliverables of the project.
Job Responsibilities:
Operational Coordination & Case Management
- Intake & Scheduling: Manage new patient referrals, intake, and eligibility determination; optimize scheduling for project team (Physician, Nurses, Nurse Assistants, Physiotherapist, Drivers) taking into account geography to maximize time and efficiency.
- Workflow Optimization: Monitor the daily delivery of services and ensure that care plans are followed.
Clinical Documentation & Compliance
- Regulatory Compliance: Support the team in maintaining compliance with palliative care standards.
- Quality Assurance: Participate in quality assurance (QA) and performance improvement initiatives.
Team & Stakeholder Communication
- Point of Contact: Act as the primary liaison between patients, family caregivers, and the project team.
- Interdisciplinary Meetings: Conduct, coordinate and participate in team meetings, recording notes and tracking action items to ensure follow-through on care plans.
- Partnership Building: Develop and sustain relationships with stakeholders such as hospitals, NGOs, community representatives, and other external service providers.
Program Support & Administration
- Project Management : Plan, implement and monitor the project for the timely achievement of the intended results having internalized the MoUs, and coordinate the project team members accordingly with the required short- and long-term plans. Conduct project planning and review meetings.
- Donor & Stakeholder Management : Do timely correspondence with donors and Partners and supportive supervision with the concerned towards judicious net utilization of funds, starting with the preparation of the annual budget and the process of getting the required funds sanctioned in time.
- Resource Management: Manage equipment, supplies, and information needed for home visits.
- Documentation Support: Prepare and submit timely project reports of activities carried out; also, the reports to the concerned Govt. agencies as and when required.
- Training Coordination: Assist in the training and orientation of new staff, volunteers, and clinicians in palliative care procedures.
Education Qualification/Required Skills & Experience
- Master’s degree in Social Work/Psychology/Sociology.
- Minimum 05 years of relevant experience in coordinating public health programs or projects preferably with a national/international NGO either at institutional or community-based model setup.
- Experience of working in a rural/urban community development projects.
- Experience of engaging with donor, partner organizations and networking organisations
- Experience using MS Windows, MS Excel and Other MS Office packages
Personal Skills
- Strong analytical and problem-solving skills, with ability to make sound judgment and decisions and offer innovative solutions
- Good presentation and facilitation skills
- Excellent communication skills
- Good in reading and writing in English and Telugu and speak in Hindi.
