Project Summary:
CHAI and Azeem Premji Foundation in collaboration have launched a new project “VASATI” for providing palliative care services in home environment in Hyderabad and Medchal Malkajgiri districts of Telangana State. The project is aimed at enhancing the quality of life and quality of death of the people living with terminal and chronic illness and their families through home visits, community awareness & education, and networking with different stakeholders.
Job Description
The incumbent will work in a team and provides expert care, pain management, and support to patients in their homes, focusing on enhancing their quality of life. The role involves managing symptoms, educating family caregivers, and end-of-life planning. Also conducts community awareness activities and supports the project team in engaging local community leaders and other stakeholders.
Job Responsibilities:
Key Responsibilities
- Clinical Care: Perform comprehensive in-home patient assessments, manage pain, administer medications, and provide wound care as needed. Implement the orders of Project Medical Officer
- Symptom Management: Monitor and manage symptoms of patients under the project.
- Family Education & Support: Educate families on care techniques, medication administration, and the end-of-life process, providing emotional and spiritual support.
- Care Planning: Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (physicians, physiotherapist, social workers and nurse assistants).
- Documentation: Maintain accurate, timely, and compliant medical records and documentation in accordance with agency policies.
- Record Maintenance: Maintain accurate, up-to-date patient files and ensure documentation complies with project and state.
- Communication: Coordinate care with doctors and other healthcare providers to ensure a seamless, patient-centered approach.
Education Qualification/Required Skills & Experience
- Registered Nurse (RN) with current state licensure.
- Bachelor’s or Diploma in Nursing from a recognized institution.
- Minimum of 2-5 years of clinical experience, preferably in palliative care, hospice, oncology, or home health.
- Demonstrated competence in symptom management and pain relief.
- Strong communication, empathy, and interpersonal skills.
- Ability to work independently and manage time effectively in a home-based setting.

- Adjusting to different home environments and rapid changes in patient conditions.
- Skilled in sensitive, honest communication with patients and families.
- Should be able to read and write in English and Telugu and speak in Hindi.
