Project Summary:
CHAI and Azeem Premji Foundation in collaboration have launched a new project “VASATI” for providing palliative care services in home environment in Hyderabad and Medchal Malkajgiri districts of Telangana State. The project is aimed at enhancing the quality of life and quality of death of the people living with terminal and chronic illness and their families through home visits, community awareness & education, and networking with different stakeholders.
Job Description
The incumbent will work in a team, will be responsible for providing comprehensive, compassionate, and specialized palliative care to patients in their own homes, ensuring pain relief, symptom management, and psychological support to improve quality of life. The role involves working closely with a multidisciplinary team (nurses, nurse assistants, physiotherapist, social workers) to manage complex symptoms and support families in navigating end-of-life. And supports the project team engage and work with local community leaders and other stakeholders in respective project sites.
Job Responsibilities:
- Clinical Care & Home Visits: Conduct regular home visits to assess patients, manage pain, and control distressing symptoms.
- Symptom Management: Prescribe medications and treatments to manage pain, nausea, breathlessness, and other symptoms associated with terminal illness.
- Patient & Family Counseling: Provide psychosocial and emotional support to patients and their families, helping them with Advance Care Planning (ACP) and end-of-life care decisions.
- Interdisciplinary Collaboration: Work with nurses, nurse-assistant, physiotherapist, social workers, and project coordinator to create and implement individualized care plans.
- Emergency Intervention: Provide phone-based consultation or, if needed, on-call emergency visits to patients in acute crisis or distress.
- Documentation & Reporting: Maintain accurate, timely, and confidential clinical records, case studies, and travel/medical logs.
- Caregiver Training: Train and mentor family members/caregivers in basic nursing care, pain management, and palliative techniques.
- Networking: Coordinate with PRATYASHA Palliative Care Centre (CHAI), local hospitals, NGOs and Community Based Organisations to facilitate referrals.
- Project Management : Participate inproject planning and review meetings; Support project team in project monitoring, data collection, and reporting.
- Undertake the assignments given by CHAI management as and when required.
Education Qualification/Required Skills & Experience
- MBBS from a recognized university with valid medical registration in Telangana.
- 3-5 years of clinical experience in a hospital, and preferably specific experience in palliative care, chronic care, oncology, or geriatrics.
- Willingness to travel extensively for home visits with the project team in Hyderabad and Medchal Malkajgiri districts of Telangana and potentially work flexible hours.
Personal Skills
- Strong commitment to dignity, comfort, and patient-centered care.
- Interpersonal skills to discuss sensitive issues regarding death and dying with family members.
- Knowledge of palliative medicine, including safe opioid prescribing (Morphine) and symptom management
- Emotional resilience to handle stressful situations, death, and bereavement.
- Good in reading and writing in English and Telugu and speak in Hindi.
