Project Summary:
CHAI and Azeem Premji Foundation in collaboration have launched a new project “VASATI” for providing palliative care services in home environment in Hyderabad and Medchal Malkajgiri districts of Telangana State. The project is aimed at enhancing the quality of life and quality of death of the people living with terminal and chronic illness and their families through home visits, community awareness & education, and networking with different stakeholders.
Job Description
The incumbent will work in a team and assists Nurse/Physician/Physiotherapist in providing expert care, pain management, and support to patients in their homes, focusing on enhancing their quality of life. The role involves supporting the team in managing symptoms, educating family caregivers, and end-of-life planning. Also the incumbent conducts community awareness activities and supports the project team in engaging local community leaders and other stakeholders.
Job Responsibilities:
Key Responsibilities
- Clinical Care: Assists the medical and para-medical team manage pain, administer medications, and provide wound care as needed. Implement orders of Nurse/Doctor under the project.
- Symptom Management: Supports medical and para-medical team monitor and manage symptoms associated with chronic or terminal illnesses (e.g., dyspnea, fatigue, nausea).
- Family Education & Support: Under the guidance of Nurse educates families on care techniques, medication administration, and the end-of-life process, and supports the team in providing emotional and spiritual support.
- Care Planning: Assists Nurse develop, implement, and update individualized care plans.
- Documentation: Maintain accurate, timely, and compliant medical records and documentation in accordance with agency policies.
- Communication: Coordinate care with other project team members to ensure a seamless, patient-centered approach.
Education Qualification/Required Skills & Experience
- Completed Intermediate/ANM.
- Preferably experienced in nursing care.
- Strong communication, empathy, and interpersonal skills.
- Ability to work in coordination with an inter-disciplinary team and manage time effectively in a home-based setting.
Personal Skills
- Providing holistic care with empathy
- Adjusting to different home environments and rapid changes in patient conditions.
- Skilled in sensitive, honest communication with patients and families.
- Should be able to read and write in English and Telugu and speak in Hindi.
