Job Overview:
Transform Rural India Foundation (TRI), in partnership with Maharashtra Village Social Transformation Foundation (MVSTF) under Mission Anadi, is supporting State and District Health Systems to strengthen prevention, early detection, referral, and continuity of care for Non-Communicable Diseases (NCDs), including hypertension, diabetes, and priority cancers.
The Health Planner – Community Mobilization will operationalise Mission Anadi’s NCD interventions at the block level by working closely with Block Health Administration, NHM structures, and SRLM platforms. The role focuses on enabling frontline health workers and strengthening community institutions to improve screening coverage, referral completion, and treatment adherence—particularly in rural, tribal, and hard-to-reach populations.
The Health Planner – Community Mobilization will be responsible for the following:
Block-level Planning & Coordination
- Work closely with the Block Medical Officer (BMO), Block Programme Manager (BPM), Block NCD Cell to prepare block-specific action plans for community-based NCD interventions
- Support alignment of Mission Anadi activities with NP-NCD, National Health Mission (NHM) and routine outreach platforms at the block level
Frontline Worker Capacity Building
- Support planning and coordination of capacity building of ASHAs, AWWs and ANMs on NCD risk identification, screening protocols, referral pathways, follow-up and counselling
- Provide on-site handholding and supportive supervision to frontline workers to improve quality and consistency of services
Community Mobilization & Engagement
- Facilitate engagement of SHGs, VOs, VHSNCs and PRI representatives to strengthen community awareness, address stigma related to NCDs and improve uptake of screening services
- Support community-level behaviour change and communication activities focused on prevention, early detection and treatment adherence
Operational Support & Field Implementation
- Travel regularly within the block to villages, Health Sub-Centres, PHCs and outreach sites to support implementation, troubleshoot operational issues and ensure adherence to planned activities
- Support organisation and monitoring of NCD screening activities integrated with VHNDs, outreach sessions and special camps
Monitoring, Documentation & Reporting
- Support block-level monitoring of key indicators such as screening coverage, referral completion and follow-up rates
- Ensure timely documentation and reporting of community-level activities to district and state teams
- Document best practices, innovations and community-led solutions, particularly from tribal and vulnerable habitations, and support replication within the block
Coordination & System Strengthening
- Coordinate with block-level NHM and SRLM functionaries and relevant departments to address bottlenecks in service delivery and referral pathways
- Support introduction of innovations for supportive supervision, digital tracking and community feedback mechanisms
Any Other Responsibilities
- Undertake any other responsibilities assigned by the Block Health Administration, District Programme Team or VSTF-TRI Mission Anadi leadership, aligned with the role
Knowledge/Skills:
- Strong understanding of community mobilisation and public health systems
- Experience engaging frontline health workers and community institutions
- Familiarity with NHM and/or SRLM structures
- Strong coordination, facilitation, and problem-solving skills
- Ability to work independently in rural and field-based environments
- Willingness to travel extensively within the block
Qualification & Experience:
- Post-Graduate degree in Public Health, Social Sciences, Management, or a related discipline
- 2–3 years of experience in public health or community development programmes
- Experience in NCD, RMNCH, nutrition, or primary healthcare programmes preferred
- Prior experience working in rural or tribal blocks will be given preference

This is a third-party payroll position and is initially set till January 2028, with monthly emoluments of INR 50,000/- only.
