Job Overview:
Transform Rural India Foundation (TRI), in partnership with Maharashtra Village Social Transformation Foundation (MVSTF) under Mission Anadi, is supporting State and local governments in Maharashtra to strengthen community-led prevention, early detection, referral, and continuum-of-care for Non-Communicable Diseases (NCDs), including hypertension, diabetes, and priority cancers.
The Neighbourhood Facilitator will be positioned at the Cluster Level Federation (CLF) level to leverage the Self-Help Group (SHG) ecosystem under NRLM as a platform for health action. The role focuses on strengthening community demand for NCD services, enabling last-mile service delivery, and driving convergence between Panchayats, SHG institutions, and the public health system.
Each Neighbourhood Facilitator will be responsible for 2–3 CLFs and will work closely with SHGs, Village Organisations (VOs), Panchayati Raj Institutions (PRIs), frontline health workers (ASHAs, ANMs, AWWs), and other local stakeholders to build replicable community-led health models.
Key Responsibilities:
Community Profiling & Planning
- Conduct Panchayat/CLF-level profiling to understand NCD burden, risk factors, service availability, gaps and vulnerable households
- Support preparation and roll-out of Panchayat/CF-level NCD Action Plans, with SHGs as key mobilising and monitoring partners
SHG-Led Community Mobilisation
- Work with SHG members, VO and CLF leadership to integrate NCD awareness, screening follow-up and treatment adherence into regular SHG meetings
- Enable SHGs to function as peer support systems for persons living with NCDs, especially the elderly and high-risk households
Frontline Worker Convergence
- Work closely with ASHAs, ANMs and AWWs to support community-level screening, referral linkage and follow-up of NCD cases
- Strengthen Village Health, Nutrition and Sanitation Days (VHSNDs) to improve coverage of blood pressure and blood sugar screening, cancer awareness and referral
Behaviour Change & Awareness
- Facilitate Behaviour Change Communication (BCC) on healthy lifestyles, nutrition, physical activity, tobacco and alcohol risk reduction through SHG forums, Gram Sabhas and community events
- Mobilise households to adopt preventive practices and self-care behaviours for long-term NCD management
Monitoring & Community Feedback
- Support community-based monitoring by SHGs and VHSNCs to track service gaps, referral completion and treatment continuity
- Identify and flag system-level bottlenecks in access to diagnostics, medicines or services and support local problem-solving with Panchayats and health teams
Documentation & Learning
- Document community-led innovations and good practices emerging from SHG-anchored NCD interventions and support peer learning across Panchayats
Knowledge/Skills:
- Understanding of community mobilisation and SHG-based institutional platforms
- Ability to facilitate group discussions and behaviour change processes
- Basic understanding of public health, NCDs, or primary healthcare systems
- Strong communication and coordination skills with community institutions and frontline workers
- Willingness to work extensively in field settings
Qualification & Experience:
- Minimum 10+2 with at least 2 years of relevant experience in health, SHG, or community development programmes
- Prior experience working with NRLM/SHG structures (VOs, CLFs) in Maharashtra preferred
- Exposure to public health, NCDs, nutrition, or primary healthcare programmes is desirable
