About the Organization:
Mamta Health Institute for Mother and Child (Mamta-HIMC), http://mamtahimc.in a not for profit institution is registered under Society Registration Act 1956. The organization was established in the year 1990 focusing on adolescent, maternal and child health. Over the past three decades, organizational work has been diversified in the areas of public health – Maternal and Child Health & Nutrition; Sexual and Reproductive Health with focus on Adolescents (10-19 years); Communicable (largely HIV, TB, and Hepatitis B & C); and Non-Communicable Diseases (Hypertension, Diabetes, Obesity & Mental Health). Nutrition being the core pillar of human development as well as a basic human need and a prerequisite to a healthy life, it is a cross cutting agenda for maximum interventions.
Purpose of the RFP:
This Request for Proposal (RFP) invites quotations from qualified BDS doctors to provide Oral and dental medical consultation services during school/community based dental Health Camps organized under the Health Initiative in Lucknow, Uttar Pradesh.
Scope of Work / Key Responsibilities
The selected Medical Officer will be responsible for the following:
• Provide general Dental consultation during 15 School and community-based Dental Health Camps
• Be available for a 3-5 hours per camp as per the mutually agreed schedule.
• Provide screening, examination, treatment and referral services to beneficiaries during camp according to standard treatment protocols or government guidelines.
• Support awareness generation on preventive health, dental hygiene, and common oral illnesses during camps.
Remuneration:
Interested Dental Doctor are requested to submit their proposed rate per health camp:

Consultancy Charges/per camp -Inclusive travel, logistics, applicable taxes, and any other incidental expenses.
Estimated number of camps: 20 throughout the project period, scheduled on mutually agreed dates.
Payment Terms
Payments will be processed upon:
• Submission of invoice.
• Satisfactory completion of each camp as verified by the reporting authority.
• Compliance with applicable tax deductions as per Government of India norms.
Tax Deduction:
Applicable taxes will be deducted at source in accordance with prevailing Government of India regulations.
Required Documents
Applicants must submit self-attested copies of the following documents:
• PAN Card.
• Aadhaar Card.
• BDS Degree Certificate.
• Valid UP Dental Council (UPDC) Registration Certificate.
General Terms and Conditions:
• This ToR is subject to programmatic needs and funding availability under the project.
• The number of camps may vary depending on local health priorities and operational feasibility.
• The Doctor must remain physically present on time at the campsite according to plan in coordination with the program manager.
• The Doctor will follow principles of medical ethics, including professional norms for protecting patient privacy and confidentiality as per the prevailing rules and regulations of the government without any breach.
• The Doctor is expected to maintain and ensure strict adherence to the POSH and POCSO policy within the work areas while interacting with beneficiaries, the MAMTA-HIMC team, and representatives of partner organizations or institutions.
• All other terms and organizational policies of MAMTA-HIMC will be applicable.
Submission of Proposal:
Proposals must be submitted via email to rfp@mamtahimc.in by 9th June 2026, with the subject line: "Proposal for Engaging Dental Doctor Lucknow". Incomplete applications and applications received after the deadline will not be considered for the selection process. The selection team will only contact applicants who have been shortlisted for the process.
Mamta HIMC has a policy of erasing all confidential information, ideas/concepts submitted in proposals/applications after the selection process is completed. "Confidentiality of Applicants" Mamta shall not be liable to share the list of shortlisted applicants with any party outside the organization. The organization takes all necessary measures to protect the confidentiality of applicants, and shall not disclose any personal information, process, and data related to the application, except as required by law.
