Brief on the Organization:
Indian Institute of Public Health Gandhinagar (IIPHG) is a University established under IIPHG Act, 2015 of Gujarat State. It offers courses in the field of Public Health. It also undertakes various research projects in the field of Public Health. The university is established by Public Health Foundation of India (PHFI) with support from the Government of Gujarat, Department of Health & Family Welfare.
Project Brief
IIPHG proposes to implement a Facility-based Intensified TB Case Finding Project across six public health facilities in Gujarat. The project will strengthen early identification of TB through systematic screening of patients attending selected OPDs and IPDs, vulnerability assessment, CXR facilitation, diagnostic evaluation, patient navigation and follow-up, and linkage with treatment, NTEP and TPT services.
The project will also strengthen facility-level implementation through standardized SOPs and workflows, healthcare-provider upskilling, Project MIS-based patient tracking and performance monitoring, supportive supervision, and documentation of operational learnings for potential institutionalization and replication.
Deliverables:
The District Coordinator will provide district-level operational leadership, coordination and supervision for implementation of project activities across assigned facilities.
Each District Coordinator will serve as the key coordination point between the State Team Lead, participating facilities, District NTEP teams, project field teams and M&E personnel. The position will ensure timely implementation of project activities, achievement of facility-level targets, effective functioning of patient-care pathways, and timely identification and resolution of operational bottlenecks.
Key Responsibilities
A. District and Facility-level Implementation
• Lead day-to-day implementation and coordination of project activities across assigned facilities.
• Translate the approved project Activity Plan into facility-level implementation schedules and operational actions.
• Coordinate facility onboarding, readiness activities and operationalization of approved workflows.
• Ensure implementation in accordance with project SOPs, operational guidelines and applicable NTEP guidance.
• Monitor facility-level progress against agreed activities, targets, deliverables, timelines and milestones.
• Provide regular implementation updates to the State Team Lead.
B. OPD and IPD Screening Operations
• Oversee systematic TB screening and digital registration across identified OPD and IPD settings.
• Ensure appropriate implementation of 10-S screening, vulnerability assessment and referral of eligible patients for CXR and diagnostic evaluation.
• Monitor functioning of facility screening points and patient flow.
• Coordinate with facility teams to ensure eligible patients identified through OPDs, IPDs, CXR/Radiology and other relevant entry points are appropriately captured.
• Identify operational barriers affecting screening coverage and facilitate timely corrective action.
C. CXR, Diagnostic and Patient Navigation Pathways
• Coordinate patient movement from screening through CXR, presumptive TB identification, specimen collection, diagnostic testing and final diagnostic decision.
• Facilitate coordination between screening teams, clinicians, radiology, laboratories and NTEP teams.
• Monitor pending CXR, specimen collection, NAAT testing, results and other incomplete diagnostic pathways.
• Ensure project field teams actively support patient navigation and follow-up to minimize diagnostic attrition.
• Facilitate linkage of diagnosed TB patients to Ni-kshay notification, treatment initiation and NTEP services.
• Facilitate referral of potential TPT-eligible individuals to NTEP for eligibility assessment and further management.
D. Field Team Supervision
• Provide direct operational supervision to Project Field Officers deployed across assigned facilities.
• Coordinate appropriate deployment of staff for screening/enrolment, patient navigation, diagnostic coordination and follow-up.
• Conduct regular team reviews and provide day-to-day guidance and problemsolving support.
• Monitor adherence to project workflows, assigned responsibilities, reporting requirements and quality standards.
• Identify capacity gaps and coordinate need-based mentoring or refresher orientation.
• Escalate significant staffing or implementation issues to the State Team Lead.
E. Facility, District and NTEP Coordination
• Maintain regular coordination with facility nodal officers, facility leadership, clinicians, nursing staff, radiology, laboratory teams and other relevant departments.
• Coordinate with District TB Centres and District NTEP teams for diagnostic, notification, treatment and programme linkages.
• Facilitate facility and district-level project review meetings.
• Ensure project-supported activities are integrated with existing facility and NTEP systems and services.
• Track decisions and corrective actions arising from facility and district reviews through completion.
F. Monitoring and Performance Review
• Work closely with District and State M&E teams to ensure timely and complete data capture in the Project MIS.
• Review facility-wise progress against screening, CXR, presumptive TB, diagnostic completion, diagnosis and treatment-linkage indicators.
• Use Project MIS dashboards and routine reports to identify pending patient pathways, performance gaps and operational bottlenecks.
• Facilitate follow-up and corrective action based on identified gaps.
• Support data validation and reconciliation with facility, laboratory and Ni-kshay records, where applicable.
• Participate in periodic data-quality assessments and performance reviews.
G. Supportive Supervision and Problem Resolution
• Conduct regular supportive supervision visits to assigned facilities.
• Identify operational issues affecting screening, CXR, diagnostics, patient navigation or treatment linkage.
• Develop and facilitate corrective actions in coordination with facility and project teams.
• Track agreed corrective actions and escalate unresolved issues to the State Team Lead.
• Facilitate sharing of effective practices and operational learnings across participating facilities.
H. Reporting and Documentation
• Prepare regular district and facility-level implementation updates.
• Maintain documentation of facility visits, stakeholder meetings, supportive supervision and corrective actions.
• Support preparation of fortnightly/monthly reports, milestone documentation and project progress reports.
• Document implementation challenges, solutions, adaptations and good practices.
• Support project close-out, facility handover and documentation of operational learnings.
• Undertake other project-related responsibilities assigned by the State Team Lead or Project Lead.
Expected Deliverable:
The District Coordinator will ensure effective operationalization of project activities across assigned facilities; functioning OPD/IPD screening, CXR, diagnostic and patient-navigation pathways; regular supervision of project field teams; coordination with facility and District NTEP teams; monitoring and resolution of operational and patient-care cascade gaps; regular supportive supervision and performance reviews; timely reporting and documentation; and achievement of assigned facility-level targets, activities and milestones.
Qualification:
Minimum: Postgraduate qualification in Public Health, Community Medicine, Hospital/Health Administration, Health Management, Social Sciences or another relevant discipline, with relevant experience in health/public health programme implementation
Desirable: Graduate degree in Medicine, Nursing, Allied Health Sciences, or another relevant discipline
Experience:
Minimum: 3 years of relevant professional experience in public health programme implementation, field coordination, healthcare programme management or related areas. Flexibility may be considered based on the candidate's qualifications and relevance of experience.
Desirable: Experience working with NTEP/TB programmes, either directly with government health systems or through academic institutions, NGOs, technical agencies or development partners. Prior experience coordinating activities across multiple health facilities will be preferred.
