About Sikshasandhan and the Programme
Sikshasandhan is a non-profit development organization working to empower marginalized and vulnerable communities through education, sustainable livelihoods, child development, health, and social development initiatives. With support from the Azim Premji Foundation, Sikshasandhan implements a community-based Creche Programme (Shishu Ghar) in Bissamcuttack Block, Rayagada, providing safe childcare, nutrition, growth monitoring, and early childhood development support to children from tribal and economically vulnerable families.

Key Responsibilities
The Health Coordinator will lead the identification, referral, management, and follow-up of child health and nutrition cases. Key responsibilities include:
• Manage identification, referral and follow up of malnutrition and illness cases.
• Identify and monitor malnutrition and illness among children enrolled in the creches, particularly Severely Underweight (SUW), Severe Acute Malnutrition (SAM), growth-faltering, and frequently ill children.
• Coordinate with CHCs, PHCs, NRCs/MTCs, and other health for medical consultation, treatment and follow up.
• Lead case management for high-risk children: several underweight (SUW, severe acute malnutrition (SAM) , growth faltering / zig zag growth, and recurring illness.
• Conduct home visits and support supervisors and cluster coordinator with home-based care and care giver counselling.
• Monitor children's growth, health status, treatment adherence, and recovery; support families in accessing appropriate health and nutrition services.
• Work closely with ASHA, AWW, ANM, and other frontline workers to strengthen health-service delivery, immunization, growth monitoring, and government scheme linkages.
• Strengthen convergence with public health and nutrition services, including immunization, growth monitoring, treatment, referrals and government entitlements.
• Support NRC/ MTC referrals, admissions, treatment completion and post discharge follow up.
• Maintain accurate records in the Shishu Ghar application and prepare periodic reports on high-risk cases, referrals, recovery and health indicators.

Qualifications and Experience
• ANM qualification from a recognized institution; higher qualifications in Nursing, Public Health, Nutrition, or related fields will be an advantage.
• 3–5 years of relevant experience in child health, nutrition, maternal and child health, community health, or malnutrition management programs .
• Experience working with public health systems, frontline health workers, and community-based health and nutrition interventions.
• Sound understanding of child growth, malnutrition identification, referral mechanisms, and government health and nutrition programs.
• Strong counselling, communication, coordination, and report-writing skills, with proficiency in spoken and written Odia and English. Knowledge of Kui, Kuvi, or other local languages will be an advantage.
• Ability to use digital applications, maintain records and prepare the basic report and undertake extensive field travel and home visits in remote communities.
Compensation
Salary will be commensurate with qualifications and experience and aligned with organizational norms.
