Educational Qualification
* Bachelor's Degree in any discipline from a recognized University.
* MSW / Social Work / Sociology / Psychology / Anthropology background will be preferred.
* Candidates having knowledge of HIV/AIDS, STI/RTI, TB, Hepatitis and other public health programmes will be preferred.
* Candidates having knowledge of government health services and referral systems will be preferred.
Experience
* Experience in health-related activities, community outreach, HIV/AIDS programmes or social sector programmes will be preferred.
* Experience of working with High Risk Groups (HRGs), vulnerable communities or marginalized populations will be an added advantage.
* Candidates having field-level experience in community mobilization, counselling, referral and follow-up activities will be preferred.
Required Knowledge & Skills
* Good communication and interpersonal skills.
* Good listening and counselling skills.
* Knowledge of HIV/AIDS prevention, STI/RTI, TB and other related health issues.
* Ability to identify and mobilize High Risk Clients/HRGs.
* Ability to conduct community outreach and field visits.
* Ability to maintain confidentiality and privacy of clients.
* Basic computer and data-entry knowledge.
* Ability to maintain records, registers and prepare reports.
* Ability to work effectively with community members, health facilities, government departments and project staff.
* Willingness to undertake regular field visits within the assigned area.
Key Roles & Responsibilities
1. Community Outreach & HRG Mobilization
* Identify, map and maintain contact with High Risk Groups (HRGs) and at-risk clients in the assigned area.
* Establish rapport and maintain regular communication with HRG community members.
* Conduct regular field visits to identified outreach sites.
* Identify new at-risk clients and facilitate their enrolment/linkage with appropriate services.
* Conduct individual and group-level awareness and behaviour change activities.
* Motivate clients for HIV, STI/RTI, TB and Hepatitis screening as per applicable programme guidelines.
2. HIV/AIDS & STI/RTI Prevention
* Provide correct information regarding HIV/AIDS, modes of transmission and prevention.
* Educate clients regarding STI/RTI symptoms, prevention and treatment.
* Conduct basic risk/vulnerability assessment and provide risk-reduction counselling.
* Promote consistent and correct condom use.
* Address myths, misconceptions, stigma and discrimination related to HIV/AIDS and STI/RTI.
* Motivate eligible clients for regular health screening and testing.
3. Counselling & Client Support
* Provide appropriate information and basic counselling to clients regarding HIV/AIDS, STI/RTI and related health services.
* Assess client needs and provide appropriate guidance.
* Maintain confidentiality of all client-related information.
* Provide follow-up support to clients after referral/testing.
* Support clients in adopting healthy and safer behaviours.
* Facilitate family/partner counselling wherever appropriate, with consent and as per applicable guidelines.
4. Referral, Linkage & Follow-up
* Facilitate referral of clients to appropriate health facilities and services.
* Coordinate/link clients with HIV Testing Services, STI/RTI services, ART, TB, Hepatitis B & C and other relevant health services.
* Follow up referred clients to ensure successful linkage with services.
* Identify clients who are lost to follow-up and facilitate re-linkage wherever required.
* Maintain proper referral and follow-up records.
5. Health Camp & Outreach Activities
* Participate in planning and implementation of approved health camps and outreach activities.
* Mobilize HRGs and eligible clients for health screening and services.
* Prepare and maintain site-wise/client-wise lists as required.
* Facilitate HIV, STI/RTI, TB and Hepatitis screening/referral activities.
* Coordinate with health service providers during camps.
* Ensure follow-up of clients requiring further investigation, treatment or referral.
6. Planning & Micro-Planning
* Prepare monthly outreach plans in consultation with the Project Coordinator.
* Identify priority outreach sites and vulnerable/at-risk clients.
* Update the field micro-plan based on client load, risk/vulnerability profile and outreach requirements.
* Work towards achieving assigned monthly targets.
* Review progress against targets and report gaps to the Project Coordinator.
7. IEC & Behaviour Change Communication
* Conduct IEC/BCC activities among HRGs and vulnerable communities.
* Distribute approved IEC materials and provide health-related information.
* Support awareness activities, community meetings, sensitization programmes and campaigns.
* Assist in appropriate display and maintenance of IEC materials.
* Promote awareness regarding HIV/AIDS, STI/RTI, TB, Hepatitis, safe practices and healthy lifestyle.
8. Commodity Management
* Monitor availability and consumption of condoms and other approved outreach commodities.
* Maintain prescribed stock/distribution records.
* Inform the Project Coordinator regarding shortage of commodities.
* Assist in timely distribution of approved commodities to eligible clients.
* Ensure proper documentation of commodity distribution and consumption.
9. Reporting & Documentation
* Maintain daily field visit records and outreach activity records.
* Maintain client, referral, follow-up and commodity-related records as prescribed.
* Ensure accurate and timely data entry in the prescribed reporting system/software, wherever applicable.
* Prepare and submit monthly reports to the Project Coordinator within the prescribed timeline.
* Provide accurate programme data and information for review meetings and reporting.
* Document good practices, success stories and important field-level interventions.
* Support preparation of presentations, reports and programme-related documents.
10. Monitoring & Review
* Participate in monthly review meetings.
* Present field-level progress, achievements and challenges.
* Review assigned targets and outreach performance.
* Implement corrective measures suggested during review meetings.
* Cooperate during programme monitoring, evaluation and field visits.
* Maintain all records required for monitoring and evaluation.
11. Coordination & Networking
* Coordinate with government health facilities and relevant service providers for referral and follow-up.
* Support convergence with relevant health and social welfare programmes.
* Coordinate with community leaders, stakeholders and partner organizations wherever required.
* Participate in advocacy, sensitization and stakeholder meetings as assigned.
* Support the Project Coordinator in strengthening linkages between HRGs and available health/social services.
12. Confidentiality & Professional Conduct
* Maintain strict confidentiality of all personal, social and health-related information of clients.
* Respect the dignity, privacy and rights of every client.
* Ensure non-discriminatory and respectful behaviour towards all community members.
* Obtain appropriate consent before sensitive follow-up/home visits wherever required.
* Follow organizational policies, programme guidelines and professional ethics.
Field Visit Requirement
The position is primarily field-based. The selected candidate will be required to undertake regular field visits to identified outreach sites and HRG locations. The candidate should be willing to travel within the assigned geographical area as required for programme activities.

General Conditions
1. The selected candidate shall work under the supervision of the Project Coordinator/Project In-charge.
2. The position requires extensive community-based and field-level work.
3. The candidate must be willing to work with High Risk Groups and vulnerable communities.
4. The candidate should be willing to travel within the assigned project area.
5. The selected candidate shall perform any other programme-related duties assigned by the competent authority.
