Project :Swasthya Setu – Integrated Community-Based Program for NCD (CVD & Diabetes) Prevention and Care

Location: District-based posting in one of the 5 project districts of Telangana: Siddipet, Rangareddy, Khammam, Nizamabad, or Nalgonda (candidates preferably resident of / already working in the assigned district will be preferred)

MAMTA HIMC is an internationally recognized Non-Governmental Organization (NGO), established in December 1990 in New Delhi. Since inception, MAMTA has promoted strategic solutions, policies, programming, research and global learning in Maternal, Newborn, Child Health and Nutrition, Young People's Sexual and Reproductive Health and Rights, HIV, TB and Non-Communicable Diseases (NCDs), with a special focus on rights, gender equality and equity. MAMTA has a presence across around 20 states in India.
About the Project
Project Swasthya Setu is an integrated, community-based program for the prevention and management of Non-Communicable Diseases (NCDs) — with a specific focus on Cardiovascular Diseases (CVD) and Diabetes — being implemented by MAMTA HIMC across five districts of Telangana: Siddipet, Rangareddy, Khammam, Nizamabad, and Nalgonda. The program targets women aged 18-30 Years(Special) and 30+(General), their families, and community institutions (VHSNCs, SHGs, PRIs) to improve NCD awareness, early detection, and long-term management by strengthening nutritional awareness, caregiver capacity, and alignment with government health systems (NHM, NP-NCD, ICDS/Anganwadi, ABDM).
The program strengthens Nutritional Awareness and Health Systems by building the capacity of frontline workers (ASHA, ANM, AWW, CHO, MPW), integrating digital/AI-based screening technology, and aligning closely with government platforms and guidelines including NP-NCD, ABDM, VHSND, and State PIP processes, in coordination with District Health Societies, NHM, and ICDS/Anganwadi systems.
Job Purpose
District Manager will be the primary point of accountability for implementation, monitoring, and government liaisoning for Project Swasthya Setu at the assigned district level. The role requires close coordination with the District Health Society, NHM, and other district-level government departments, along with oversight of field teams, community mobilization, and NCD (CVD,Diabetes) screening and referral activities.
Roles and Responsibilities
Program Implementation & Field Oversight
• Lead end-to-end implementation of Project Swasthya Setu activities at the district level, ensuring adherence to project timelines and technical guidelines.
• Conduct monthly program reviews and support quarterly monitoring of NCD screening, referral, and adherence-tracking activities.
• Supervise and mentor block-level field staff, Peer Leaders/Volunteers, and community mobilizers engaged under the project.
• Ensure saturation of project villages/blocks as per the program's phased scale-up strategy (Villages ? Blocks ? District).
• Oversee use of digital/AI-based screening devices, Mobile MIS, and patient-tracking systems for BP, diabetes, and related NCD indicators.
Government Liaisoning & Coordination
• Build and maintain strong working relationships with the District Health Society, NHM, District Medical & Health Officer (DM&HO), ICDS/Anganwadi structures, and other relevant district government departments.
• Represent MAMTA HIMC in district-level review meetings, health department coordination meetings, and NP-NCD/NHM platforms.
• Facilitate integration of project activities with government systems, including support for beneficiary entry into NP-NCD and ABDM digital health records.
• Support alignment of project deliverables with district and State Program Implementation Plans (PIPs).
• Obtain necessary permissions and approvals required for facility-level and community-level project activities.
Capacity Building & Technical Support
• Coordinate training and continued handhold support of ASHAs, ANMs, AWWs, CHOs, and Medical Officers on NCD (CVD & Diabetes) screening, referral pathways, and adherence support.
• Strengthen referral pathways across the care continuum — Community (ASHA/ANM/AWW) ? HWC/PHC ? CHC ? Specialist Referral (SDH/District Hospital).
• Support formation and orientation of Peer Leaders/Volunteers, and capacity building of VHSNCs, Panchayats, SHGs, and CBOs/CSOs for community ownership of NCD prevention.
• Work with content/technical experts on nutrition and lifestyle counselling components, including dietary awareness aligned with local Telangana food practices.
Monitoring, Reporting & Documentation
• Maintain the project MIS with accurate, updated data on beneficiaries, screenings, referrals, and events conducted under the project.
• Prepare timely monthly and quarterly progress reports, case studies, and district-level updates for the State/National team.
• Conduct periodic quality audits/spot-checks of a sample of facilities and field activities each month.
• Track and report on program indicators, flagging implementation gaps and supporting course-correction/pivoting as required.
• Undertake any other responsibilities related to the project as assigned by the Senior Project Coordinator.
Qualification and Skills required
• Master's degree in Public Health, Social Work, Social Sciences, Nursing, Rural Development, or a related field.
• 2–6 years of relevant work experience, with mandatory prior experience working in Telangana state with the District Health Society/NHM department, preferably within one of the project districts (Siddipet, Rangareddy, Khammam, Nizamabad, or Nalgonda).
• Demonstrated knowledge of and prior experience working on NCD programs, specifically related to Cardiovascular Disease (CVD), Diabetes prevention, screening, or management.
• Strong government liaisoning skills, with an established understanding of district health administration, NHM structures, and NP-NCD/ABDM frameworks.
• Fluency in Telugu (spoken and written) is mandatory; working proficiency in English is required for reporting and documentation.
• Good working knowledge of MS Office, particularly MS Word, MS Excel, and MS PowerPoint; familiarity with digital MIS/health data platforms preferred.

• Ability to independently manage district-level government relationships and represent the organization credibly with health department stakeholders.
• Strong people-management skills to lead, mentor, and supervise field-level staff and community volunteers.
• Excellent analytical and problem-solving skills, with the ability to identify and resolve field-level implementation challenges.
• Willingness to travel extensively within the assigned district, including to rural and remote facilities, on a regular basis.
• High level of diplomacy, integrity, and confidence in engaging with government officials, community institutions, and MAMTA's internal teams.
• Commitment to gender-sensitive, rights-based, and community-participatory approaches to public health programming.
