Key Responsibilities:

• Tracking and follow-up of MIS & LFU cases and facilitating treatment.

• Providing support services to newly initiated PLHIV clients.

• Follow-up of PPW cases for ANC check-up, institutional delivery, treatment adherence & prevention of vertical transmission.

• Follow-up of infants for EID testing and HIV-free certification.

• Linkage of ICTC reactive clients with ART Centre for treatment initiation.

• Follow-up of Sero-discordant couples, 2nd/3rd line ART and ATT cases.

• Support PLHIV for treatment adherence, retention and health services.

• Facilitate social entitlement support and documentation.

• Conduct field visits and other tasks assigned by CSC Coordinator.

Qualification & Skills:

• Minimum 12th Pass.

• Basic knowledge of NACP and willingness to work with PLHIV community.

• Good communication and documentation skills.

• Preference will be given to candidates from PLHIV community or affected groups.
