Role Summary
The Center Coordinator is responsible for the effective day-to-day functioning of the Day Care Centre, ensuring holistic support for elders through program management, health coordination, documentation, and community engagement, while upholding dignity, respect, and professional standards of care.
Key Responsibilities
- Support elders in daily needs including hygiene, nutrition, emotional well-being, and basic health assistance.
- Facilitate counselling support and promote active ageing through cognitive, recreational, yoga, meditation, and skill-based activities.
- Plan, implement, and monitor structured daily activity schedules at the Centre.
- Organize monthly doctor consultations and quarterly health camps.
- Coordinate medical referrals and ensure timely follow-ups for elders requiring additional care.
- Conduct regular weekly home visits to assess living conditions, identify needs, and strengthen engagement with elders and families.
- Maintain accurate and updated member records, including profiles, ID documentation, registers, and case notes.
- Ensure all records are organized, confidential, and audit-ready at all times.
- Support elders in accessing government schemes, social entitlements, and appropriate legal aid services.
- Liaise with local authorities, healthcare providers, NGOs, and service partners.
- Collaborate effectively with internal teams, volunteers, and community stakeholders.
- Conduct weekly review meetings every Friday to track activities, challenges, and outcomes.
- Report key updates and field insights to the Programme Manager.
- Prepare and submit Monthly, Quarterly, Half-Yearly, and Annual reports in a timely manner.
- Work closely with the Program Manager, Community Mobilizer, Helper, and volunteers.
- Maintain a professional, empathetic, and respectful approach in all interactions with elders and stakeholders.
Required Qualifications & Experience
- Graduate / Post-graduate in Social Work, Psychology, or related field
- 3–5 years’ experience in helpline, NGO, healthcare, or social sector work
- Experience working with senior citizens or caregivers preferred
Skills Required
- Good verbal and written communication
- Empathy and patience while handling calls
- Ability to manage a small team
- Basic computer skills (MS Office / data entry)
- Ability to handle stress and emergency situations
