Key Responsibilities:

Key Functions: Community Liaison for Health (CLH) will lead the field activities of the CSC. They will identify and facilitate registration of PLHIV including those with special needs (children, HRGs, ANC) at the CSC. In conjunction with peer Navigator, they will support the PLHIV to access health and non-health services through regular follow up.

Specific roles and responsibilities of Community Liaison for Health (CLH):

oIdentification of new PLHIV and registering them for CSC.
oConduct field visits and participate in the following awareness generation activities:
1.Conduct home visits for the registered PLHIV and affected families to provide treatment education, education on OIs, nutrition, healthy lifestyles and positive living, and psychosocial support. Support of peer counsellor can be taken for addressing specific issues during home visits.
2.Track LFUs (ART and pre-ART) and report back on the status. Based on the list of LFU and MIS cases received from the ART centre, plan home visits for the defaulters, and motivate them to resume their treatment. Where feasible, accompany them to the services. For untraced defaulters, report the status to CSC Co-ordinator with evidence.
3.Liaise with and conduct regular visits to respective ART centres assigned by the coordinator.
4.Conduct PPW follow-ups to eliminate Vertical Transmission, ensuring treatment adherence, ANC check-ups, institutional delivery, and post-natal care.
5.Facilitate linkage of ICTC identified reactive clients to the nearest ART Centre, ensuring prompt treatment initiation.
6.Monitor PLHIV on 2nd or 3rd line ART, focusing on various health outcomes and reducing transmission risk.
7.Conduct follow-ups for Sero discordant couples.
8.Follow-up on infants, administering life-saving drugs, and adhering to the EID test schedule for up to 18 months for a HIV-free certification.

oProvide information on home based care to PLHIV and their family members.
oDevelop weekly and monthly work plans, including details of activities such as home visits, LFU follow-up, advocacy events, visit to health facilities, meetings in CSC, and participation in support group meetings.
oUndertake advocacy by doing the following:
a.Organizing community events with the help of peer counsellors and DLN (e.g., health camps, fairs, festivals).
b.Organizing and participating in advocacy activities, including rapport building with other stakeholders both within government and in the community.
c.Liaise with Anganwadi worker (AWW) and ASHA for referral of eligible children and pregnant women to access supplementary nutrition.
d.Liaise with community leaders, local clubs and other social organizations to enhance their involvement in the program.
Facilitate filling of forms for social entitlements and schemes and hand them over to the CSC Co-ordinator.
