Background:
Severe Acute Malnutrition (SAM) is one of the greatest challenges to child survival in the world today. Affecting 16 million children worldwide and responsible for up to two million child deaths each year, it is the most lethal form of malnutrition. Not only does it cause rapid and severe weight loss – leaving children physically wasted – it seriously and significantly compromises their immune system, making it almost impossible to resist disease. Severely wasted children are, on average, 11 times more likely to die than their healthy counterparts. It thus becomes critical to have an effective and efficient SAM management programme in the state equipped to reach the most vulnerable children with a focus on gender and equity.
In Bihar, severe acute malnutrition is a major health concern. NFHS-6 shows the prevalence of wasting at 19% and severe wasting at 4.7%. Both indicators have decreased from 22.9% and 8.8% in NFHS-5; however, the levels remain very high and a cause of concern. The wasting prevalence is very high in the first 6 months after birth, with NFHS-5 data showing it to be 31%, with severe wasting at 15 %. Thus, providing care and management for these children is very important for reducing the incidence and prevalence of undernutrition.
UN ORGANISATION is an active partner of the Government of Bihar (GoB) for SAM management in partnership with the Centre of Excellence, Patna Medical College and Hospital (PMCH), Department of Community & Family Medicine, AIIMS and Dr Rajendra Prasad Central Agriculture University (DRPCAU)-PUSA. Currently, the government of Bihar operates Nutritional Rehabilitation Centres (NRCs) in all districts and at 3 Medical colleges for facility-based treatment of children with SAM. The government has also scaled up Community Management of SAM to all districts of Bihar, in line with the MWCD guideline, with support of UN ORGANISATION.
The Samagra Shishu Baal Swasthya Karyakram (SSBSK) has recently been launched by MoHFW to identify early growth faltering and children at nutritional risk. In 2026-27, the focus will be on strengthening community management of SAM across the state for children under 5 years of age, with an intensified focus on addressing early growth failure in children under 6 months through the rollout of the SSBSK programme. Under the overall guidance and technical supervision of the Nutrition Officer, UN ORGANISATION BFO, the Consultant will provide technical support to the State Health Society, Bihar, and ICDS for the planning, implementation, and monitoring of the SAM programme in the State and its districts.
