Job Description:
• Plan goal-oriented counselling session with each child as per individual needs.
• Record the counselling sessions
• The summary and key highlights of each counselling session to be documented in the child case file
• Maintain the Track Child Portal
• Assist in developing the Individual Care Plan
• Contribute your opinion on the individual care plans and counselling records
• Provide inputs on each child being counselled during the Home Management Committee Meetings.
• Submit monthly progress report at HO.
• Participate in admission, restoration and follow up process.
