1. Background:
Assam has achieved measurable gains in maternal health over the past decade, with maternal mortality declining by nearly 190 points; however, the state continues to be the fourth-highest maternal mortality ratio in the country. While coverage indicators have improved (NFHS-5, 2019–20), quality and effective utilization of services remain uneven. Nearly one-fifth of women are married before 18 years, and despite an eight-percentage-point decline, teenage pregnancy remains above five percent among girls aged 15–19 years. Only 50.7 percent of pregnant women received four or more ANC visits, 84.1 percent of deliveries occurred in health facilities and integration of HIV services remains weak, with just 16.6 percent of women tested for HIV during ANC. These gaps contribute to an estimated 550 maternal deaths annually, underscoring the need to strengthen care across pregnancy, childbirth and the post-partum period.
The Government of Assam is implementing multiple national and state flagship programmes, including Pradhan Mantri Surakshit Matritva Abhiyan (PMSMA), Assam Arogya Nidhi, Boat Clinics and targeted high-risk pregnancy identification initiatives. While these interventions have expanded access, challenges persist in quality, referral linkages, follow-up and equity, particularly in hard-to-reach and vulnerable populations. Despite statewide rollout of the Mother and Child Protection (MCP) card, effective use for counselling and tracking remains limited. In parallel, Maternal Death Surveillance and Response (MDSR) systems require further strengthening through systematic reviews, improved data quality and actionable analysis to inform responsive planning and accelerate reductions in preventable maternal deaths across Assam.
