About the Organization:
Registered in 2001 under the Bihar Societies Registration Act, Samagra Seva is a Jamui-based grassroots organization founded by former child labourers empowered through ChildFund India. Driven by lived experience, the team works in the most remote areas of Jamui—including Khaira, Gidhaur, Jhajha, and the Bhimbandh forest region—delivering education, health, and dignity to Mahadalit (Musahar), tribal, and minority communities struggling with poverty and forced migration.

About the Role:
The Health Coordinator in creche initiative is responsible for ensuring timely identification, follow-up, and management of malnutrition and illness among children enrolled in creches. The role involves coordinating with health facilities and frontline workers, supporting referrals and treatment adherence, conducting home visits for high-risk children, and counselling families on nutrition and child health. The Health Coordinator also strengthens convergence with public health services and maintains accurate records of referrals, follow-ups, and health outcomes to support effective case management and child recovery.

Key Responsibilities
a. Take overall responsibility for the identification, management, and follow-up of malnutrition and illness cases among children enrolled in approximately 40 creches.
b. Coordinate with Community Health Centres (CHCs), Primary Health Centres (PHCs), Nutrition Rehabilitation Centres (NRCs), and supervisory teams to facilitate scheduled medical consultations, referrals, and treatment services. Ensure pre-referral planning and facility readiness for smooth service delivery.
c. Lead the follow-up and case management of high-risk children, including those identified as Severely Underweight (SUW), Severe Acute Malnutrition (SAM), children with growth faltering (zig-zag growth patterns), and children with recurring illnesses.
d. Conduct regular home visits for high-risk children and provide technical support to Supervisors and Cluster Coordinators in home-based malnutrition management and caregiver counselling.
e. Monitor treatment adherence, medicine consumption, dietary compliance, creche attendance, growth trends, and overall health status of high-risk children.
f. Provide counselling and guidance to parents and caregivers on nutrition, illness management, treatment compliance, hygiene, and child care practices.
g. Work closely with frontline health and nutrition functionaries, including ASHAs, Anganwadi Workers (AWWs), Auxiliary Nurse Midwives (ANMs), and facility-based health staff to ensure timely support for children.
h. Strengthen convergence with public health and nutrition services to facilitate access to entitlements, treatment, growth monitoring, immunization, and other essential services.
i. Counsel and support families of children referred to NRCs/MTCs and ensure timely admission, treatment completion, and post-discharge follow-up.
j. Maintain accurate and up-to-date records of referrals, follow-ups, treatment outcomes, and child health status in the Shishu Ghar application and other prescribed reporting systems.
k. Prepare periodic reports on high-risk cases, referrals, recoveries, and key health indicators, and share updates with program and supervisory teams for timely decision-making.
Educational Qualification & Experience
a. Auxiliary Nurse Midwifery (ANM) qualification from a recognized institution. A higher qualification in Nursing, Public Health, Nutrition, or a related field will be an added advantage.
b. Minimum 3–5 years of field experience in child health, nutrition, maternal and child health, community health, or malnutrition management programs.
c. Demonstrated experience in working with public health systems, frontline health workers (ASHA, AWW, ANM), and community-based health and nutrition interventions.
d. Sound understanding of child growth and development, malnutrition identification and management, referral mechanisms, and government health and nutrition programs.
e. Strong interpersonal, counselling, and communication skills, with the ability to effectively engage with caregivers, community members, and health personnel.
f. Proficiency in Hindi and Odia (spoken and written). Knowledge of additional regional languages will be an advantage.
g. Ability to maintain accurate records, use digital applications for data entry and reporting, and prepare basic program reports.
h. Willingness to undertake extensive field travel and conduct regular home visits in remote communities.

Remuneration:
Competitive monthly salary commensurate with qualification and experience, along with travel allowances for field visits.
