PURPOSE OF POST:

Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSFI protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions.

FUNCTIONAL AND HIERARCHICAL LINES

Hierarchically accountable & functionally accountable to Medical Activity Manager.

ACCOUNTABILITIES:
• Apply medical knowledge and skills to diagnose and prevent conditions relevant to the projects work. Carry out outpatient consultations, prescribing the necessary treatment respecting MSFI protocols.
• Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
• Follow up the evolution of the hospitalized/IDP patients, through daily visits , consultations and examinations, prescribing the necessary treatment following MSFI protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution.
• Check and control the rational distribution of medicines and equipment under his/her responsibility and monitor the quality, disinfection and sterilization of the medical material. Assure compliance with IPC standards.
• Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator of any important trends. Inform line manager of any problem or complication of the patients’ illness or medical error, as per protocols.
• Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care.
• Knows and ensures all MSFI medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing biohazard risks and improving infection control. Ensures professional confidentiality is respected.
• Manage the team under his/her direct responsibility according to MSFI HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.

Context Specific Accountabilities:
MSFI currently provides HIV, TB and Hepatitis C treatment in MSFI clinics in two districts of Manipur: Chandel (Chakpikarong) and Tengnoupal (Moreh). MSFI provides OST (Opioid Substitute Therapy) in OSCC (One Stop Care Centre) in Moreh (Tengnoupal). MD will mainly work in Karong and Moreh, and in outreach clinics as operationally needed.

Roles and Responsibilities of MD at MSFI HIV-TB-HCV Clinics Karong, Moreh and outreach:
• Provide diagnostics and treatment of HIV, TB (DS/DRTB), HCV, STI, NCD, and other medical conditions following MSFI Project specific Protocols at MSFI Clinics.
• Work closely with other members of medical team staff (Nurses for triage and patient flow, Counselors, Health Promoters, Lab Technicians and Medical Data Processing Officers) to ensure quality medical care.
• Accurate filling of all required medical forms in the patient file and collection of clinical data as required
• Ensures patient’s confidentiality is respected.
• Refer patients to appropriate facilities (CCPur District Hospital, Moreh SDH, Tertiary Hospital in Imphal etc.) for further investigations and treatment or special care based in indications set by MSFI Protocols; and follow up the evolution of referred cases.
• Participate in mobile clinics to remote locations, as required by the project.
• Ensure proper and careful use and check quality of biomedical equipment following the user manual and protocols and alert supervisor in case of malfunctioning.
• Support clinic/community activities with any additional tasks assigned.
• Inform MAM of any irregularities, shortage or damage of materials, medical incidents (by submitting incident report to line manager)
• Participate in clinical and medical management team meetings.
• Participate in any emergencies/explorations if necessary.
• Close collaboration and teamwork with all other med/ para med/non med departments to implement and achieve project road map objectives.
• Can be delegated by MAM as responsible for ensuring proper allocation of medical reimbursements in line with medical reimbursement policy
• Actively participate in medical training sessions and clinical case reviews
• Provide inputs for final compilation of monthly medical report analysis.

Roles and Responsibilities of MD at the OSCC
• The MD plays a lead role in the medical aspects of OST program. He /she is expected to provide the best possible medical care for OST along the MSFI/NACO guideline.
• The MD will deal with PWIDs and OST patients without discrimination, in line with Medical Ethics.
• Ensure patient’s confidentiality is respected at all times.
• MD will perform the following duties following MSFI/NACO OST & HIV/HCV/STI treatment Guidelines:

1. Assess patients for suitability of initiation into Opioid Substitution Therapy (OST)
2. Provide medical/mental support to PWIDs, motivate to stop using IVD and switch to OST and to be adherent to OST
3. Induce, Maintain and Terminate the OST along the MSFI/NACO Guidelines.
4. Prescribe suitable doses and individualize Buprenorphine dose appropriately on patients
5. Assess complications and conduct regular follow ups for OST
6. Manage overdose with Naloxone if it needs and document it as per the NACO guideline
7. Conduct Mental Status Examination (MSE) in PWIDs to detect mental illness and refer to facility with a higher level of care
8. Provide screening, diagnosis, and treatment of HIV/TB/ HCV/HBV, STI/SRH, NCD cases, send sample to MSFI Clinic for further confirmation.
9. Initiate HCV/HIV/TB, STI treatment in MSFI OSCC Clinic. Organize patients’ treatment follow-up.
10. Provide medical treatment for common clinical conditions including wound and abscess care
11. Carry out timely referrals of patients in need to the appropriate facilities, after discussion with MAM
12. Communicate clearly with clients and family members about the illness and the management process involved and ensure they have understood.
13. Assist in compiling monthly medical reports.
• Any other tasks appointed by line manager as per project medical needs

REQUIREMENTS :
• Education: Essential: MBBS degree with full & valid registration from Medical council of India or state medical councils, Desirable diploma in Tropical Medicine
• Experience: Essential Minimum 1 year of work experience
Desirable experience with HIV, (DR) TB and HCV
• Languages: Mission- English (Essential) and Local language (Desirable)
• Knowledge : Essential computer literacy (word, excel)
