1. Introduction & Background
AFSL as part of its Corporate Social Responsibility (CSR) mandate under, invites proposals from experienced and registered Non-Governmental Organizations (NGOs) to plan, organize, and implement General Health Checkup Camps in rural and semi-urban communities across Indian states.
The primary objective of this initiative is to promote preventive healthcare and facilitate the early diagnosis of common lifestyle diseases among underserved populations. Through this project, we aim to bridge the gap in healthcare access in remote areas and improve the overall health and well-being of community members.
This RFP is specifically issued for NGOs with demonstrated experience in community health mobilization, implementation of health camps, and coordination with government health machinery at the block/district level.
2. Project Scope & Objectives-
2.1 Overall Objective
To conduct General Health Checkup Camps in multiple blocks/districts across 10 states, ensuring access to quality preventive healthcare services for at least 150–200 participants per camp.
2.2 Scope of Work for the Implementing NGO
The selected NGO shall be responsible for end-to-end planning and implementation of health camps, including but not limited to:
A. Camp Setup & Coordination:
• Identifying suitable camp venues in coordination with local authorities, panchayats, or community leaders.
• Setting up temporary, well-organized health checkup camps at different venues across designated blocks/districts.
• Conducting screening for a minimum of 150–200 persons per camp.
• Ensuring adequate seating, water supply, and hygiene at the camp site.
B. Medical Staffing:
• Deploying qualified MBBS doctors and registered nurses.
• Ensuring the availability of supporting project staff for monitoring and implementation.
C. Health Screening Services:
The following diagnostic and consultation services must be provided at each camp:
Blood Pressure (BP) Measurement
Hemoglobin (Hb) Testing
Body Mass Index (BMI) Assessment
Random Blood Sugar Testing
Electrocardiogram (ECG)
General Physician Consultation
D. Beneficiary Support:
• Providing on-the-spot health reports and basic medical advice to each participant.
• Distribution of essential medicines as required.
E. Community Awareness & IEC Activities:
• Conducting Information, Education & Communication (IEC) activities including wall paintings, leaflets, and banners on common health issues.
• Organizing public announcements (PA system) to mobilize community members prior to and during camps.
F. Compliance & Reporting:
• Ensuring proper bio-medical waste disposal in compliance with applicable health regulations.
• Submitting a comprehensive report for camps, including participation data, demographic details, screening findings, and a summary of health observations.
• Providing geo-tagged photographs and beneficiary lists as part of documentation.
G. Travel & Field Coordination:
• The project team must be prepared to travel to different blocks/districts as per the mutually agreed implementation schedule.
• The NGO must ensure local accommodation and transport arrangements for field staff as budgeted in the financial proposal.
3. Eligibility Criteria
Interested NGOs must fulfil all of the following eligibility conditions to be considered for selection:
1) Legal Registration: Must be a registered NGO/Trust/Society under applicable Indian laws (e.g., Societies Registration Act, Indian Trusts Act, Section 8 of Companies Act 2013).
2) Tax & CSR Compliance: Must possess valid 80G and 12A certifications from the Income Tax Department, and must have filed CSR Form 1 with the Ministry of Corporate Affairs (MCA).
3) Relevant Experience: Must have a prior experience in organizing or implementing community health camps or similar public health outreach programs.
4) Healthcare Credentials: Must have access to or partnerships with licensed medical professionals (MBBS doctors) and registered nursing staff.
4. Proposal Requirements
Each proposal must be comprehensive, well-structured, and include all of the following components:
1) Organization Profile: Brief overview of the NGO including history, mission, legal registrations, key leadership, and areas of work.
2) Proposed Methodology & Approach: A detailed description of how the NGO plans to organize and execute the health camps, including community mobilization strategy, camp coordination process, and quality control mechanisms.
3) Proposed Health Tests & Equipment: List of all diagnostic tests to be offered and equipment to be deployed at each camp.
4) Financial Proposal: Complete cost breakdown as per Annexure 1 (Cost Per Camp for 150 persons). All costs must be itemized and justified.
5) Value-Added Services: Any additional services, community programs, or innovations the NGO proposes to enhance the impact of the camps.
6) Supporting Documents: Copies of registration certificates, 80G, 12A, CSR Form 1, PAN card, last 3 years' audited financial statements, and any relevant MOU/empanelment letters.
5. Evaluation Criteria
All received proposals will be evaluated by a designated selection committee based on the following weighted criteria:

6. Submission Instructions
Interested NGOs are requested to submit their complete proposal (Technical + Financial) strictly via email only. No physical submissions will be entertained.

7. Terms & Conditions
• The issuing organization reserves the right to accept or reject any or all proposals at any stage without assigning any reason.
• All information shared as part of this RFP and the submitted proposals will be treated as strictly confidential and used only for evaluation purposes.
• Submission of a proposal does not guarantee selection or award of the project.
• The final implementation schedule, number of camps, and locations will be mutually agreed upon between the issuing organization and the selected NGO after the selection process.
• The selected NGO must comply with all applicable laws.
• The issuing organization may conduct due diligence, site visits, or reference checks as part of the selection process.
• Any canvassing or lobbying by the applicant NGO will result in immediate disqualification.
• Payment terms will be as per the contract/MOU signed between the parties after selection.

• The NGO cost management fee is capped at 5% of Total Project Cost (Row B).
