Qualifications: Intermediate to graduate from a recognized Academic institute.
Desirable:1 year experience of working with community.

Job requirements: •To participate in awareness generation activities along with other teammates with special focus on community mobilization. 
• To support and supervise ASHAs’ in screening of general population with use of validated tools
• To do community/facility based assessment of screened population. 
• To refer screened cases to block service points for further assessment and care.
• Regularly follow up plans of clients registered on the program. 
• Hold regular case management sessions with clients and care givers as per the program protocols. 
• To make home visits as part of case management protocols.
• To help clients gain access to support groups, day care facilities, certifications, higher education, vocational skills and employment facilities, reservations and other benefits as per program protocols.
• To facilitate access to education for children in the family, old age pension for elders in the family etc. to reduce the overall burden of care and improve quality of life for patients and their families.
• To ensure documentation and timely reporting as per requirement of the program.
• To supervise and handhold ASHAs’ to perform their tasks in program.
• To undertake any other activity as per the guidance of the reporting manager and/or needs of the program.
