Request for Proposals to undertake Baseline District-level Assessment to inform a family-centered TB and nutrition intervention in Vaishali, Bihar
Introduction:
Resource Group for Education and Advocacy for Community Health (REACH), established in 1999, is a non-profit organization dedicated to the fight against TB in India. REACH’s mandate includes providing high-quality treatment, care, and support for those affected by TB as well as advocacy, research and education to improve public understanding of TB. REACH works closely with the National TB Elimination Program, TB-affected communities, private providers, and other civil society groups and acts as an interface between the public and private healthcare systems.
Project background:
REACH is currently implementing a family-centred nutrition and TB care model in Vaishali district, Bihar. The project aims to improve nutritional status, enhance TB treatment outcomes, and reduce household-level transmission by supporting both people with TB (PwTB) and their household contacts through nutritional support, counselling, and linkages to existing public health and social welfare systems.
Purpose of this call
REACH is looking to engage a research agency to undertake a baseline assessment on TB and nutrition, using mixed methods, in Vaishali district, Bihar with the following objectives:
- Establish baseline nutritional status and vulnerabilities: To conduct baseline nutritional assessment on Anthropometric indices such as body mass index, mid upper arm circumference; prevalence of comorbidities such as HIV, Diabetes mellitus; substance use such as tobacco usage, alcohol usage or others among the People with TB (PwTB) and their household contacts.
- Evaluate Knowledge, Attitude and Practice, Health and Nutrition Vulnerabilities and Access, and Treatment Experiences: To assess KAP related to TB and nutrition, nutritional vulnerabilities, health-seeking behaviour, household food insecurity, access to support systems among PwTB and their household contacts, and treatment experiences, key barriers and facilitators to treatment completion and recovery among PwTB in Vaishali district.
- Map dietary practices and preferences: To understand local dietary practices, food preferences, and consumption patterns to inform the design of nutrition baskets for PwTB and their households.
- Synthesize evidence on TB and nutrition: To compile and synthesize evidence from published literature and case studies on relation of TB and undernutrition and the various nutrition interventions for TB affected households globally and nationally.
Methodology
- Type of study: We expect the agency to adopt a mixed-methods approach with quantitative cross-sectional surveys and qualitative enquiries among TB-affected households.
- Participants in the survey: Eligible participants include People with TB (PwTB) with their current facility in District Vaishali as well as their household contacts. The PwTB will be selected from a line list of previously (completed treatment within the last 6 months) and currently (currently at least completed 3 months of treatment) treated publicly notified PwTB.
- Sample Size:
- The sample size for the quantitative survey among PwTB has been calculated using n = 4pq/d², taking the prevalence of underweight among PwTB as 82.4% (RATIONS trial). With 10% relative precision and 10% non-response, the final sample size is estimated at approximately 95.
- The sample size for the household-level assessment has been calculated using the formula n = 4pq/d², taking the prevalence of underweight among household contacts of PwTB as 34.2% (RATIONS study). Assuming an absolute precision of 6% and allowing for 10% non-response, the final sample size is estimated at approximately 275 households. (This is an indicative number, and the agency can suggest a different sample size with due justification.)
